| heal.abstract | The aim of the study was to record the cases of sudden cardiac death in the region of Epirus during the period 1998-2008, so as to estimate the objective incidence, and to investigate their etiology. We recorded consecutive sudden death cases in the population 1-80 years of the region of Epirus (353,820 inhabitants) referred for autopsy to the Department of Forensic Medicine and Toxicology of University of Ioannina, Greece, in the period January 1998- December 2008 (11 years). Cases of sudden death in infants and in the elderly >80 years and cases whose the toxicological analysis was positive for toxic substances and drugs of abuse (cocaine, heroin, alcohol, etc.) were excluded. 688 cases were recognized as sudden cardiac deaths, aged 1-80 years. The estimated annual frequency of sudden death due to cardiac causes, in the population of Epirus was 18,6/100,000 (approximately 63 annual deaths). The 549 (79,8%) subjects were males and the 139 (20,2%) females, mean age 59,5 ± 12,1 years and 68,3 ± 11,5 years, respectively. Death was ascribed to ischaemic heart disease in 564 (82%) of the total population. In 41,7% an acute ischaemic event was identified, in 28,4% there was myocardial scarring but no acute ischaemic event and 29,9% had coronary atheroma only. Death was due to left ventricular hypertrophy in 62 (9%), to cardiomyopathies in 25 (3,6%) and in 18 (2,6%) death was unexplained. Among total population, 28 (4,1%) cases (17 men, mean age 27,9 ± 7,9 yrs; 11 women, mean age 21,6 ± 8,9 yrs) belonged in the age group <35 years. The estimated incidence of sudden cardiac death in the population 1-35 years in the region of Epirus was 1,78/100,000 persons of young population (<35 years) per year (approximately 2 annual deaths). The major causes of death in the age group 1-35 years were atherosclerotic coronary artery disease in 5 cases (17,8%), myocarditis in 3 (10,7%) and cardiomyopathies in 3 (10,7%) cases (2 hypertrophic cardiomyopathy, 1 dilated cardiomyopathy), whereas in 11 cases (39,3%) death was unexplained. - 92 - Sudden unexpected cardiac death especially in the young is still an unresolved medical problem. Relatives need to be referred for cardiological and genetic screening in cases of normal hearts found at autopsy.
